RN Clinical Case Manager

RN Clinical Case Manager supervises home care personnel and ensures compliance with state/federal regulations while coordinating each patient’s Plan of Care. Responsibilities include training and re-educating clinical staff, direct supervision of field RNs/LPNs/LVNs, and providing guidance to patients and families. Role requires timely clinical documentation, incident reporting, participation in quality improvement activities, and effective communication with Clinical Supervisor/Director of Clinical Services. The position supports the mission of the company, maintains professional appearance, adheres to policies, and involves patient- and family-centered care across home health services. Must hold an active RN license, with BSN preferred, and possess a current CPR credential and valid driver’s license. Relocation assistance is not indicated.
