Job Summary
Community-based RN Care Manager coordinating care for dually-eligible enrollees with complex medical, behavioral, and social needs. Conducts home/community assessments (including MDS-HC), leads the interdisciplinary care team, develops and implements care plans, navigates community resources, educates members and families on benefits and preventive care, supports care transitions, and ensures continuity of care across providers in Massachusetts with regular travel to member and provider sites.
Required Qualifications
- Associates of Science (A.S) degree in nursing or Master’s degree in social work or mental health counseling and independent license required
- Current unrestricted clinical license in the Commonwealth of Massachusetts as a Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), Licensed Independent Clinical Social Worker (LISCW), or Licensed Mental Health Counselor (LMHC) required
- Case Management Certification is highly preferred
- Must have valid driver's license, vehicle and verifiable insurance
- Influenza vaccination is a requirement of this position and proof of flu vaccination annually is required
