Job Summary
The Clinical Care Leader (CCL) provides individualized nursing care using the nursing process in a home-setting care context, collaborating with inter-professional colleagues to plan, implement, and evaluate care. Responsibilities include documenting resident history, symptoms, medications and care, supervising unlicensed personnel, delegating and leading department initiatives, and maintaining accountability for resident assignments. Requires strong communication, clinical reasoning, and ability to coordinate care across disciplines, with emphasis on age/population-specific standards and adherence to Nurse Practice Act and applicable regulations. A BSN is strongly preferred; an RN license (and BLS) is required, with 40-hour weekly shifts and opportunities for advancement and comprehensive benefits.
Required Qualifications
- Unencumbered RN license in the state of practice or multi-state licensure via Nurse Licensure Compact (NLC) if applicable
- Basic Life Support (BLS) certification
- Bachelor's degree in Nursing strongly preferred (BSN) or equivalent
- Minimum two years of clinical experience preferred
- Licensure and competencies aligned with state practice act and standards
- Possession of department-specific competencies and certifications as required by location
