Clinical Care Leader (RN) provides individualized nursing care in the home setting by applying the nursing process (assessment, diagnosis, planning, implementation, evaluation). Collaborates with inter-professional colleagues to plan, implement, and evaluate care; documents resident history, symptoms, medications, and care in compliance with standards; utilizes technology to enhance resident care; delegates to and supervises unlicensed personnel; assumes nursing accountability for resident assignments; participates in leadership tasks and committees; demonstrates nursing expertise with appropriate population-specific standards; communicates effectively with residents, families, and peers; maintains applicable competencies and certifications; may require valid driver’s license and BLS; RAI process experience is preferred.
