Job Summary
Telephonic RN Clinical Care Coordinator coordinating post-acute care for home health, hospice, and palliative services. Responsibilities include evaluating patient needs via medical records and discussion, matching patients to appropriate service sites or alternative services, acting as patient advocate through the referral process to maximize timely admissions, following up on referrals and outcomes, adhering to HIPAA and company policies, providing education on Home Health, Hospice, and Palliative Care, and assisting with admissions and special projects. Requires an Associates Degree, RN license in a compact state, 3+ years of post-acute nursing experience, CPR certification, and strong communication and problem-solving skills; remote, part-time telephonic role with standard nursing responsibilities.
