Job Summary
Assess patients' physical, psychosocial, cultural, and spiritual needs through observation, interviews, and medical record reviews. Develop and implement individualized plans of care while evaluating patient responses to ensure optimal outcomes. Demonstrate competency in therapeutic interventions including IVs, wound care, medication administration, and specimen collections. Provide patient and family education based on learning readiness and maintain accurate, timely documentation of status and interventions. Collaborate with the interdisciplinary team to promote patient safety and advocate for health and rights. Utilize information technology to communicate effectively and support decision-making. Requires current state RN license, BLS certification, and experience in ambulatory care and case management.
Required Qualifications
- Associate Degree in Nursing
- Graduate of an accredited School of Nursing
- Current state RN license
- Current RN license from a participating state in National Licensure Compact
- BLS
- Ability to exert up to 50lbs force occasionally
- Ability to exert up to 20lbs frequently
- Ability to exert up to 10lbs constantly

- One (1) year experience
