Job Summary
RN Certified Diabetes Care & Education Specialist (Per Diem) — deliver evidence-based diabetes education to patients and families, develop/revise education materials, promote self-management to achieve individualized goals, participate in the Diabetes Care Committee, review and implement ADA Standards of Medical Care, and educate across ambulatory clinic settings. Responsibilities include counseling patients, documenting in medical records, leading educational activities and group presentations, applying adult learning principles, collaborating with nursing and clinical leadership, and monitoring program effectiveness. Requires Nursing degree (Associates, Bachelor preferred), CA Nursing license, two years of clinical nursing and two years diabetes education experience, and CDCES certification within six months of hire (exam eligible with 90-day completion). Bilingual Spanish and HIPAA-compliant handling of protected health information are desirable.
Required Qualifications
- Associates Degree in Nursing required
- California Nursing license
- Certification of Diabetes Care and Education Specialist (CDCES) required within 6 months of hire
- CDCES exam eligible will be considered and must pass within 90 days of start
- Minimum of two (2) years of clinical nursing experience and two (2) years of diabetes education experience (inpatient and outpatient)
- EHR experience preferred
- Bilingual Spanish preferred
- Basic life support (BCLS) current
