Job Summary
RN Case Manager for Hospice coordinates interdisciplinary patient-centered care using the nursing process and evidence-based practice. Works within WellStar’s governance model to ensure high-quality, safe patient care, with emphasis on teamwork, care planning, education for patients and families, and ongoing professional development. Requires a BSN (accredited program), RN licensure (GA or multi-state compact), and BLS; minimum 10 years of direct patient care experience.
Required Qualifications
- Graduate of an accredited/approved school of nursing
- Bachelor's Degree Baccalaureate degree in nursing (BSN) from an accredited school of nursing
- Reg Nurse (Single State) or RN - Multi-state Compact
- Basic Life Support or BLS - Instructor
- Minimum Experience: Minimum 10 years of current direct patient care experience
- Ability to read, write and speak English; strong interpersonal and collaboration skills; capable of performing in fast-paced environments and responding to emergencies; strong time management; evidence-based nursing practice and analytical skills
