Job Summary
RN Case Manager Hospice role at Wellstar; coordinates interdisciplinary nursing care, leads nursing process (assessment, diagnosis, planning, implementation, evaluation) to provide individualized, high-quality, safe patient care in hospice; emphasizes relationship-based, patient-centered care, interdisciplinary rounds, and evidence-based practice; requires BSN, RN licensure (prefer multi-state compact), BLS, 10+ years of direct patient care, strong communication and teamwork within a fast-paced healthcare setting; location: Marietta, GA.
Required Qualifications
- Graduate of an accredited/approved school of nursing
- Bachelor's Degree in Nursing
- RN license (Single State) or RN - Multi-state Compact
- Basic Life Support (BLS) - Instructor
- Minimum 10 years of current direct patient care experience
- Strong interpersonal, collaborative, and customer service skills
- Fluent in English; ability to read, write and speak English; proficient with technology
