Job Summary
RN Case Manager Hospice role focused on delivering patient-centered, evidence-based nursing care within an interdisciplinary team. Responsibilities include applying the Nursing Process to assess, diagnose, plan, implement, and evaluate patient care; coordinating care with partners through interdisciplinary rounds and clear documentation; ensuring culturally competent, individualized care; mentoring and precepting other staff; promoting safe, high-quality outcomes and participation in performance improvement initiatives; supporting data collection, research utilization, and adherence to organizational policies and patient safety goals. Requires BSN education and RN licensure (GA or multi-state Compact), BLS, and at least 10 years of direct patient care experience along with strong communication and computer proficiency.
Required Qualifications
- Bachelor's Degree in Nursing (BSN) from an accredited school of nursing
- Registered Nurse (RN) license; GA state license or multi-state compact
- Basic Life Support (BLS) certification
- Minimum 10 years of direct patient care experience
- Strong interpersonal, collaborative, and customer service skills
- Proficiency with technology and electronic health records
- Ability to read, write and speak English; effective communication
- Ability to work in a fast-paced environment and respond to emergencies
- Ability to plan and manage time effectively
- BSN or higher preferred for some roles; required minimum education explicitly listed as BSN
