Job Summary
RN Case Mgr Hospice at Wellstar seeks an experienced RN to lead and participate in an interdisciplinary team delivering patient- and family-centered hospice care. Responsibilities include applying the nursing process to planning and evaluating care, coordinating services across team members, and promoting evidence-based practices, patient safety, and high-quality outcomes. Requires a BSN from an accredited program, current RN license (preferably multi-state compact), BLS certification, and a minimum of 10 years of direct patient care. The role emphasizes collaboration, education of staff and families, and adherence to professional practice standards within a WellStar governance framework. Location: Kennesaw, GA. Shift: Various US-based shifts.
Required Qualifications
- Bachelor's Degree in Nursing (BSN) from an accredited school of nursing
- RN license to practice in Georgia or multi-state compactRN license
- Basic Life Support (BLS) certification
- Strong English communication skills (written and spoken)
- Minimum 10 years of current direct patient care experience
- Ability to read, write and speak English; proficient with technology and documentation systems
- Ability to function in a fast-paced environment and respond to emergencies with decisiveness and composure
- Strong interpersonal and collaborative skills; customer service orientation
- BSN-prepared preferred for senior clinical leadership roles
