Job Summary
RN Case Manager for MultiCare Hospice in Thurston County coordinates and delivers interdisciplinary care plans to move patient progress toward clinical, quality, and cost-effective outcomes. Responsibilities include organizing patient care using the nursing process, collaborating with the nursing team to improve overall quality of care and patient experience, and advocating for ethical, holistic care that respects patient autonomy and diverse backgrounds. The role requires a Bachelor’s degree in Nursing (or equivalent within five years), an active WA RN license, BLS, MLS endorsement, and 3 years of recent clinical experience in a specialty area; an advanced nursing degree (MSN/PhD/DNP/ARNP) is preferred.
Required Qualifications
- Bachelor's degree in Nursing (or obtained within five years from initial date of hire)
- Current Registered Nurse license in Washington State or Multistate License endorsement (MLS)
- Current BLS Certification
- Three (3) years of recent clinical experience in specialty area
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
