Job Summary
RN Case Manager at MultiCare Hospice in South King County. Responsibilities include organizing patient care by prioritizing, delegating, delivering and evaluating; applying the nursing process to deliver care per hospital policies; collaborate with the nursing team to improve quality of care, patient experience and cost reduction; foster a learning environment for patients, families, nursing staff and health-care team; advocate for ethical and holistic care promoting autonomy, dignity and health of patients from diverse backgrounds.
Required Qualifications
- Bachelor's degree in Nursing (or obtained within five years from hire)
- Current Registered Nurse license in Washington State or MLS endorsement
- BLS Certification
- Three (3) years of recent clinical experience in specialty area
- Advanced degree in Nursing (MSN, PhD, DNP, ARNP) preferred
