RN Case Mgr - Highlands Case Management - FT - 1st Shift

Review medical records for SI/IS to determine admission appropriateness, continued stay necessity, and ancillary service use. Coordinate discharge planning and social services functions while ensuring compliance with federal, state laws, and Joint Commission standards. Liaise with community agencies, physicians, and other departments to ensure patients receive recommended follow-up services. Maintain accurate documentation and professional rapport across all stakeholder groups. This role supports Utilization Review and Discharge Planning activities within Highlands Medical Center's 170-bed facility in Scottsboro.
