Job Summary
RN Case Manager at Western North Carolina Community Health Services in Asheville, NC. Provide care management to Medicare/Medicaid patients at Minnie Jones Health Center, collaborating with medical, behavioral health, pharmacy, dental, and care management teams to develop customized care plans and ensure successful transition of care from inpatient or emergency department to culturally competent primary care. Requires NC RN licensure, BLS, a Bachelor’s degree in nursing or related field, and two years of RN experience with Medicaid experience; experience in community health or outpatient care preferred, bilingual in English and Spanish and/or Russian a plus, and HIV care experience a plus. Compensation based on 40 hours/week with a base salary of $75,000 to $80,000; benefits include health plans, telehealth, 403b, holidays, PTO, life insurance, and CME support.
Required Qualifications
- NC state RN licensure
- BLS certification
- Bachelor’s degree in nursing or related field
- Two years’ RN experience
- Experience with Medicaid patients
