Job Summary
As a Registered Nurse Case Manager, provide 1:1 holistic in-home care for hospice patients and families, partnering with an interdisciplinary team to develop individualized care plans, guide end-of-life conversations, and ensure comfort with professionalism and empathy. Responsibilities include completing initial and ongoing comprehensive assessments, administering medications and treatments, educating patients and caregivers on care techniques, medications, and symptom management, and rotating on-call duties to ensure seamless care transitions. Must have reliable transportation, valid driver’s license, and proof of auto insurance. Schedule is 8:00am–5:00pm, Full-time with on-call expectations. Required Qualifications
- Associates, Bachelors, or Masters of an accredited nursing program (RN)
- 1+ years of recent acute care experience; hospice/home health experience preferred
