RN Case Manager - Weekends

Job Summary
Review patient hospitalizations, recommend appropriate levels of care, and confirm clear documentation in medical records. Act as a liaison between emergency department physicians, hospitalists, attending physicians, and post-acute or third-party payers to facilitate multi-disciplinary communications. Collaborate with the care team to develop and implement discharge plans based on individualized patient needs while overseeing functional and financial outcomes. Perform pre-admission, admission, and continued stay reviews per CMS and payer guidelines. Maintain knowledge of utilization review standards and ensure compliance with federal, state, and local laws.
Required Qualifications
- Graduate of an accredited school of nursing
- Current registered nurse license in state of practice
- Basic Life Support (BLS) from the American Heart Association
- Ability to work a flexible work schedule (e.g. 24/7, weekend, holiday, on call availability)
- Proficiency in using computers, software, and web-based applications
- Effective verbal and written communication skills and ability to present information clearly and professionally to varying levels of individuals throughout the patient care process
- Strong interpersonal skills
- Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects
- Duties performed routinely require exposure to blood, body fluid and tissue
- Occupational risk for exposure to all communicable diseases
- Occupational risk for exposure to hazardous medications or hazardous waste within the environment
Desired Qualifications
- Bachelor's degree in nursing
- 3 years of hospital-based experience in discharge planning, case management or utilization review
- Certification in Case Management (CCM) or Accredited Case Manager (ACM)
