Job Summary
RN Case Manager coordinating in-home patient care and case management across northwest Georgia and southeast Tennessee. Responsibilities include applying nursing skills to assess patient needs, develop care plans, document accordance with state requirements, and collaborate with medical staff to ensure quality, age-appropriate patient care—primarily geriatric. Requires clinical nursing proficiency, ability to work independently in patients' homes, strong communication, and the capacity to travel to various locations daily. Must hold an active Georgia (or multi-state) RN license, BLS/CPR, and a valid driver's license with appropriate auto insurance.
Required Qualifications
- Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing
- Current Georgia or multi-state license as a Registered Nurse
- American Heart Association BLS CPR required
- Valid driver's license and automobile insurance coverage
