RN Case Manager (Visits)

Job Summary
RN Case Manager (Visits) responsible for conducting in-person patient interviews and assessments, overseeing plan-of-care implementation with care teams (home health aides, LPNs, RNs), communicating patient status with providers, educating patients/families on home health care procedures (e.g., wound care, IV administration, med management), and working to decrease readmissions through preventive care and continuity of care. Requires current RN license, CPR/BLS/First Aid, reliable transport, and at least 1 year of RN experience in home health or similar settings; OASIS experience preferred; trach/vent experience preferred but not required. Full-time/part-time/PRN/weekends with flexible scheduling and daily pay option offered by Interim HealthCare; equal opportunity employer.
Required Qualifications
- Current unrestricted RN license in the state of employment
- Current CPR/AED/BLS/First Aid certification
- Reliable transportation to/from care sites/work locations
- 1 year RN experience in home health or similar setting; case management/utilization review preferred
- OASIS experience preferred
- Practical tracheostomy/ventilator experience preferred but not required
