RN Case Manager (Visits)

RN Case Manager (RNCM) for Home Health Visits: conduct in-person patient interviews and comprehensive assessments, oversee implementation and ongoing assessment of the patient’s plan of care via supervision of home health aides, LPNs, RNs, and other caregivers; communicate patient conditions and collaborate with providers as needs evolve; educate patients and families on home health care procedures (e.g., wound care, IV administration, medication management); work to decrease readmissions by promoting preventative care and ensuring continuity of care. Flexible scheduling (Full-time/Part-time/PRN/Weekends) with daily pay option; no overtime required.
