RN Case Manager (Visits)

Conduct in-person patient interviews and comprehensive physical assessments while overseeing home health aide, LPN, and RN care plans. Communicate evolving patient conditions to providers and educate families on wound care, IV administration, and medication management protocols. Work to decrease readmissions by promoting preventative care and ensuring continuity of care across the home health setting. Requires current unrestricted RN licensure, CPR/AED/BLS/First Aid certification, and one year of home health experience; OASIS and trach/ventilator experience preferred. Full-time, part-time, PRN, and weekend shifts available with daily pay options and no overtime required.
