RN Case Manager- Visiting Hospice Nurse

RN Case Manager for Visiting Hospice Nurse position coordinating patient care, interpreting and explaining patient rights and Hospice Medicare benefits, conducting ongoing physical and psychosocial assessments, following the plan of care, collaborating with the IDT, supervising hospice aides/LPN staff, facilitating equipment procurement, documenting and reporting changes, providing patient/family teaching, and participating in on-call rotations; requires nursing license and hospice care experience with strong communication, documentation, and infection control practices.
