RN Case Manager- Visiting Hospice Nurse

RN Case Manager- Visiting Hospice Nurse provides direct patient care in home hospice; coordinates care per plan, interprets Medicare Hospice Benefit, and educates patients and families. Responsibilities include ongoing physical and psychosocial assessments, following established care plans, collaborating with the IDT, supervising hospice aides/LPN staff, facilitating equipment needs, documenting vital signs and treatments, nursing documentation and charting within policy, maintaining infection control, participating in education programs, and rotating on-call duties. On-site role with schedule Mon-Fri 8:30a-4:30p in NJ (Newton); sign-on bonus included.
