Job Summary
Coordinate discharge planning to assure patient progression through the continuum of care and ensure discharge to the least restrictive environment. Review diagnostic results and level-of-care needs while collaborating with physicians and multidisciplinary teams to advocate for patients and ensure timely, appropriate care. Maintain compliance with quality patient care and regulatory standards. Hours are assigned based on clinical and operational needs, with typical shifts including Monday-Friday, 8:00am-4:00pm and occasional weekend rotation. A sign-on bonus up to $10,000 is offered, scaling based on experience and employee status.
Required Qualifications
- Minimum of two (2) years of nursing experience
- Graduate of an accredited nursing program
- Licensed/registered RN in NY State
- Maintain current professional licensure in nursing or professional filed of certification
Desired Qualifications
- Bachelor Nursing degree
- Experience in a hospital Case Management/discharge planning
- Appropriate certification in the case management (e.g. Commission for Case Management Certification (CCMC) or Association of Rehabilitation Nurses)
