RN Case Manager (UTICA, NY)

Coordinate discharge planning to ensure patients progress through the continuum of care and are discharged to the least restrictive environment. Review diagnostic results and level-of-care needs while collaborating with physicians and multidisciplinary teams to advocate for appropriate patient care. Assess patients across the continuum of care and help ensure timely, right care. Maintain compliance with quality patient care and regulatory standards. This role supports the expanding Case Management team at MVHS, focusing on clinical decision-making and meaningful impact on patient outcomes. Hours are assigned based on clinical and operational needs, with typical shifts including Monday-Friday, 8:00am-4:00pm and occasional weekend rotation.
