RN Case Manager coordinates patient care within the Healthcare & Medical Services department. In this full-time role, you will perform comprehensive assessments, develop and manage discharge plans, and guide patients and families through the continuum of care. You’ll collaborate closely with physicians and interdisciplinary teams, ensuring appropriate utilization of services and smooth care transitions. Required Qualifications
- RN license
- nursing licensure in good standing
- case management experience (preferred)
- ability to coordinate care plans and collaborate with interdisciplinary teams
- strong communication and assessment skills
