Job Summary
Coordinate and oversee home-based patient care as an RN Case Manager. Provide skilled nursing care including assessments, medications, and treatments; counsel patients and families in their residence per physician orders and plan of care under the direction of the RN Clinical Manager. Schedule is full-time, Monday to Friday, 8:00 am - 4:30 pm, with paid drive time and a company vehicle; no weekends or holidays and limited on-call. Benefit offerings include health insurance, 401k with match, flexible spending accounts, tuition reimbursement, PTO, and ongoing training. Requires Wisconsin RN license, at least two years of nursing experience with preference for home health, CPR certification, and a valid Wisconsin driver's license and auto insurance.
Required Qualifications
- Active Wisconsin Registered Nurse License
- Two (2) years nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- A valid Wisconsin Driver's License and Car Insurance
