RN Case Manager

Provide expert guidance and oversight in clinical and regulatory matters, including staff training, care plan review, incident investigation, and coordinating services for patients in their homes. Counsel patient families in accordance with physician orders and plans of care under RN Clinical Manager supervision. This full-time role operates Monday through Friday with limited on-call rotation.
