Job Summary
Transition at Home RN Case Manager coordinates and oversees patient-centered home health care, providing staff training, care plan review, incident investigation, and counseling for patients and families at the patient’s residence under the direction of the RN Clinical Manager.
Required Qualifications
- Active Wisconsin Registered Nurse License
- Two (2) years’ nursing experience, 1 year in Home Health (preferred)
- CPR Certification
- Wound Care would be beneficial
- A valid Wisconsin Driver's License and Car Insurance
