RN Case Manager

RN Case Manager provides skilled nursing care in patients' homes, including assessments, medications, and treatments; coordinates services and counsels patients and families in place of residence under physician orders and plan of care, supervised by the RN Clinical Manager. Full-time hours are 8:00 am–4:30 pm, Monday through Friday with no holidays or weekends and a limited on-call rotation.
