Job Summary
Provide skilled nursing care to patients in their homes, coordinate care with the interdisciplinary team and referring agencies, and oversee ongoing assessments, planning of care, and education for patients and families. Responsibilities include implementing individualized plans of care, coordinating services with the IDT, communicating patient needs and changes to physicians, documenting progress, and performing on-call duties as assigned. Requires nursing expertise, strong communication, ability to travel locally, and comfort with home-based and end-of-life care.
Required Qualifications
- Current state RN license
- BSN/ASN/ diploma nursing graduate (as applicable)
- Valid driver's license
- Ability to travel locally to patient homes
- Proven interpersonal communication skills
- Prolonged sitting/walking; lift up to 50 lbs as needed
- Background in hospice or home health preferred (2 years experience preferred)
