Job Summary
Coordinate comprehensive participant care in collaboration with an interdisciplinary team, conducting initial, semi-annual, annual, and change-of-condition assessments per CMS guidelines. Administer medications including oral, topical, IV, and injectable forms while managing medication-related processes and verifying reconciliation. Develop and maintain accurate nursing care plans, attend IDT and Plans of Care meetings, and facilitate discharge planning from hospitals or skilled nursing facilities. Complete in-home visits for nursing services outside the scope of LVN In-home Service nurses and determine nursing levels of care as delegated. Maintain participant medical records, fulfill CESD charting requirements, and communicate updates regarding contact information changes. Work full-time Monday through Friday, 8:00 AM to 4:30 PM.
Required Qualifications
- Current California Registered Nurses License
- CPR certification
- A minimum of one year's experience in working with the frail or elderly population
- Valid driver's license
- Reliable transportation
- Ability to clear fingerprints through DOJ and FBI
- Ability to pass a drug screen clearance
