Job Summary
RN Case Manager for St. Paul’s PACE in San Diego. Full-time role coordinating participant care with interdisciplinary team, administering medications, conducting assessments, developing and updating care plans, and facilitating in-home nursing visits in accordance with CMS guidelines. Requires CA RN license, CPR, 1+ year experience with elderly population, and valid driver’s license. Travel to participant locations as required; Monday–Friday schedule, typical 8:00 AM–4:30 PM. Required Qualifications
