Job Summary
The RN Case Manager leads the hospice care team to create and manage each patient’s Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff; provides day-to-day patient care, administers medications, maintains patient records, coordinates care with physicians and interdisciplinary teams, ensures HIPAA compliance, and participates in on-call duties; requires RN license, nursing education, and ability to travel to home-based settings; starting base salary $89,000–$98,000 with comprehensive benefits.
Required Qualifications
- Current Registered Nurse (RN) license in state(s) of practice
- Graduate of a professional school of nursing
- Ability to pass DHS background, consumer report, and drug test
- Valid State Driver’s License and access to automobile in good working order
