RN Case Manager

RN Case Manager leads each patient’s hospice care team to develop and manage the Plan of Care in collaboration with the Hospice Medical Director, patient, family and facility staff; provides day-to-day patient care, administers medications, maintains patient records, coordinates care with physicians and interdisciplinary teams, educates patients and families, ensures HIPAA compliance, and supports activities of daily living. Additional duties include on-call visits (evenings/weekends), attending interdisciplinary meetings, supervising ancillary personnel, engaging in professional development, contributing to quality assessment, and performing other duties as assigned.
