RN Case Manager

Conduct 1:1, whole-person care for patients and families in their homes by completing initial and ongoing comprehensive assessments and developing individualized care plans. Administer medications and treatments as prescribed while educating caregivers on care techniques, medications, diet, and symptom management. Partner with an integrated care team to coordinate care and support families through complex decisions, rotating on-call duties to ensure seamless transitions. Requires 1+ years of acute care experience, reliable transportation, and a valid driver's license. Full-time position with an 8:00am to 5:00pm schedule.
