RN CASE MANAGER - SPRING VALLEY (FULL-TIME)

Job Summary
Establish safe, individualized discharge plans and provide proficient, timely utilization management services to ensure maximum reimbursement for all patient visits. Collaborate proactively with patients, families, and the healthcare team to achieve quality outcomes while addressing social and physical factors affecting functional status. Leverage knowledge of community resources to meet post-discharge clinical and social needs, utilizing computer proficiency for data collection and management. Requires a current Nevada RN license, three years of varied clinical experience, and two years in utilization review or case management.
Required Qualifications
- Graduate of an accredited school of nursing
- A minimum three years experience in varied clinical settings
- Current license to practice as a Registered Nurse in the State of Nevada
- Knowledge of social and physical factors that affect functional status at discharge
- Knowledge of community resources to meet post discharge clinical and social needs
- Computer proficiency to include word processing, spreadsheet, and data collection/management computer programs
- Must be able to demonstrate the knowledge and skills necessary to provide care/service appropriate to the age of the patients served on the assigned unit/department
Desired Qualifications
- Two years experience in Utilization Review, Utilization Management or Case Management
