Job Summary
Coordinate the nursing plan of care for a caseload of approximately 15 patients with varying acuity, providing direct nursing care and instructing LPN and Hospice Aide staff. Assess patients' physical and mental health, record assessment results, and plan, initiate, deliver, and evaluate appropriate nursing acts and treatment regimens. Teach, assign, or supervise personnel in implementing treatment regimens while collaborating with healthcare providers to determine patient-centered plans. Report nursing care given and patient responses, supervise nursing functions, and maintain safe and effective care standards. Work Monday through Friday with rotating emergency weekend/holiday call, requiring a valid NC RN license and two years of clinical experience.
Required Qualifications
- Valid NC RN license
- Valid Driver's License and proof of automobile insurance
- Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
- Knowledge of HIPAA privacy rules and regulations
- Knowledge of electronic medical records systems
- Demonstrate basic data entry skills
- Demonstrate excellent organization skills
- Demonstrate excellent interpersonal skills
- Demonstrate excellent written and oral communication skills
- Ability to work under pressure to meet deadlines
- Ability to take initiative and utilize innovative techniques and ingenuity

- Demonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation
- Must be willing and able to care for patients with infectious disease processes unless approved accommodations
- Assist in providing 24 hour on-call services, including coverage of at least one holiday per year
Desired Qualifications
- Two years of clinical experience preferred
