RN - Case Manager South Region

Job Summary
Coordinate the nursing plan of care for a caseload of approximately 15 patients with varying acuity, providing direct nursing care and instructing LPN and Hospice Aide staff. Assess physical and mental health, record results, and plan, initiate, and evaluate appropriate nursing acts while teaching and supervising personnel. Collaborate with healthcare providers to determine patient-centered plans and implement prescribed treatment and pharmaceutical regimens. Report care plans, nursing interventions, and patient responses, ensuring safe and effective care delivery. Work Monday through Friday with rotating emergency weekend and holiday calls, maintaining knowledge of federal, state, and local regulations including Medicare Conditions of Participation and HIPAA privacy rules.
Required Qualifications
- Valid NC RN license
- Valid Driver's License and proof of automobile insurance
- Knowledge of Federal, State and Local regulations and Medicare Conditions of Participation
- Knowledge of HIPAA privacy rules and regulations
- Knowledge of electronic medical records systems
- Demonstrate basic data entry skills
- Demonstrate excellent organization skills

- Demonstrate excellent written and oral communication skills
- Ability to work under pressure to meet deadlines
- Ability to take initiative and utilize innovative techniques and ingenuity

- Demonstrate understanding of reimbursement, regulatory requirements, and Medicare Conditions of Participation
- Must be willing and able to care for patients with infectious disease processes unless approved accommodations
- Assist in providing 24 hour on-call services, including coverage of at least one holiday per year
Desired Qualifications
- Two years of clinical experience preferred
