RN Case Manager- Sign-on Bonus - $7,500

RN Case Manager in Bloomington, IN is responsible for conducting in-person patient interviews and comprehensive physical assessments, overseeing the implementation and ongoing assessment of the patient’s plan of care with home health aides, LPNs, RNs and other caregivers, communicating patient conditions and collaborating with providers to adjust care as patient needs evolve, and educating patients and families on proper home health procedures (e.g., wound care, IV administration, medication management). The role aims to decrease readmissions by promoting preventative care and ensuring continuity of care, with 1:1 patient care and flexible scheduling (Full-time/ Part-time / PRN / Weekends).
