Job Summary
Provide patient outreach, education, and care coordination in clinic, community, and patient home settings. Assess barriers to care such as transportation, food access, housing, insurance, medications, and health literacy. Connect patients with medical, behavioral health, social service, and community resources. Collaborate with providers, care teams, social workers, and community partners. Document outreach, referrals, care plans, and patient progress accurately and timely. Work five 8-hour days with no nights, weekends, or holidays. This community-based nursing role helps patients overcome barriers to care, connect with needed resources, and improve health outcomes within a collaborative, mission-driven environment.
Required Qualifications
- Current nursing license in the state where services are provided
- RN
- BLS/CPR certification
- Valid driver's license
- Reliable transportation
- Ability to travel locally as needed
- Strong communication skills
- Strong organization skills
- Strong documentation skills
- Strong teamwork skills
- Must be available for five 8-hour days
- Must not work nights
- Must not work weekends
- Must not work holidays
