Job Summary
The RN Case Manager is responsible for initiating, developing, and managing the patient Plan of Care (POC), establishing patient-specific goals, and providing quality patient-centered care in the patient's home. Candidates must have a minimum of 1 year experience as a Registered Nurse, a valid RN license, and Current CPR/BLS Certification. Responsibilities include regular communication with the care team regarding patient status, ensuring accurate documentation, and delivering rehabilitative nursing services.
