Job Summary
RN Case Manager for Home Health: performs initial patient and family assessments, initiates and revises the plan of care, reevaluates patient needs, determines nursing diagnoses, develops care plans addressing therapeutic, preventive, and rehabilitative actions, administers medications as prescribed, counsels patients and families, provides education, identifies discharge planning needs, and coordinates home health services across Santa Cruz County (Felton, Boulder Creek, Ben Lomond). Requires active RN license, BLS/CPR, valid driver’s license, and 1 year of acute care and/or home health experience; full-time role with M-F schedule and weekend on-call rotation. Hybrid work arrangement is indicated.
Required Qualifications
- Current and valid state RN license
- Minimum of 1 year of recent acute care experience in an institutional setting
- Basic Life Support (BLS), with hands-on CPR certification
- Valid driver's license and auto insurance
- Associate's or Bachelor’s degree in Nursing, and 1 year of home health experience preferred
