RN Case Manager / RNCM (Home Health)

Conduct initial patient and family assessments to determine home care needs, including complete physical examinations and illness histories. Initiate and revise plans of care based on changing patient status, incorporating therapeutic, preventive, and rehabilitative nursing actions. Re-evaluate patient needs regularly, use health assessment data to establish nursing diagnoses, and develop care plans with goals involving the patient and family. Administer prescribed medications and treatments, counsel patients on meeting care plans, provide health care instructions, and implement discharge planning prior to patient release.
