RN Case Manager / RNCM (Home Health)

Conduct initial patient and family assessments to determine home care needs, including complete physical examinations and illness histories. Initiate and revise plans of care based on changing patient status, utilizing health assessment data to establish nursing diagnoses. Develop care plans with therapeutic, preventive, and rehabilitative actions, incorporating patient and family input. Administer prescribed medications and treatments, counsel patients on meeting care goals, and provide appropriate health care instructions. Identify and implement discharge planning needs prior to patient release. Requires current RN license, acute care experience, BLS certification, and valid driver's license. Pay range $45-$50 plus PPV/NVA/ROC fees; PRN schedule available in Carson City, Nevada.
