RN Case Manager

Coordinate services and develop individualized care plans for patients as an RN Case Manager. Guide patients and families through orientation, rehabilitation philosophy, discharge planning, Medicare and insurance benefits, and patient rights. Collaborate with physicians, nurses, and the interdisciplinary team to ensure safe transitions, clear communication, and consistent quality across the care continuum. Perform psychosocial assessments, educate patients and families, and document findings to support treatment planning and family conferences. Promote compassion, respect, and excellence in every patient and family interaction.
