Job Summary
Build meaningful relationships with patients and families by understanding their goals, barriers, and motivators. Create personalized care plans that address social determinants and emotional well-being beyond diagnoses. Coordinate care across providers and settings to ensure safety, quality, and continuity while educating members to navigate the healthcare system confidently. Collaborate with an interdisciplinary team to advocate for whole-person health and use data to identify opportunities while maintaining regulatory compliance. This remote role requires a minimum of five years as a Registered Nurse with an active Compact RN license and strong skills in motivational interviewing. Candidates must be local to St. Tamany Parish, LA.
Required Qualifications
- Minimum of 5 years as a Registered Nurse
- Active Compact RN License
- Strong background in care management as well as complex and chronic condition management
- Skilled in motivational interviewing, patient education, and patient advocacy
- Ability to work independently in a remote environment while staying connected to the team
- Comfort with technology and multiple EMR systems
- Must be local to St. Tamany Parish, LA
- Minimum of 100 mbps download, 10 mbps upload internet connection
