Job Summary
RN Case Manager oversees and coordinates hospice patient care after admission, collaborates with interdisciplinary team to manage plan of care, monitors pain and symptoms for patient comfort, facilitates admissions and referrals, serves as a preceptor for nurses and trainees, participates in performance improvement and community outreach, and rotates on-call to provide 24-hour service to patients and families.
Required Qualifications
- RN licensure in Arkansas
- BSN preferred
- one year nursing experience in relevant settings
- valid Arkansas driver's license and auto insurance
