RN Case Manager (Registered Nurse)

The RN Case Manager oversees and participates in clinical interventions and support for an assigned hospice patient caseload, coordinating and updating the Plan of Care with the interdisciplinary team, communicating with patients, families, and other healthcare providers, and ensuring timely documentation and adherence to policies. Responsibilities include monitoring pain and symptom control, coordinating CNA scheduling, initiating referrals for hospice-related services, serving as a preceptor for nurses and students, participating in care planning and performance improvement activities, engaging in community outreach about hospice services, and providing 24-hour on-call coverage as assigned. Requires RN licensure in Arkansas; BSN preferred; valid Arkansas driver’s license and automobile insurance required. This role emphasizes knowledge of palliative care, effective communication, teamwork, safety, confidentiality, and the ability to operate in a fast-paced environment.
