Job Summary
RN Case Manager / Recertification Nurse for Home Health – NW Ohio travel-focused leadership role. Manages patient care across starts of care, admissions, recertifications, transitions, and ongoing assessments (OASIS/NON-OASIS), supervises nursing staff, collaborates with physicians and care teams, ensures compliant documentation, and coordinates patient status with the care team. Field leadership responsibilities include supervising RN/LPN/HHA and coordinating with payors, with travel across multiple counties in Northwest Ohio and a weekday schedule (8am–5pm) with no overnights. Competitive full-time compensation ($78,000–$83,200 annually) plus potential bonuses, mileage reimbursement, and benefits. Requires nursing degree, active Ohio RN license, and willingness to travel; CPR/First Aid; strong organizational and communication skills; adherence to HIPAA and regulatory guidelines.
Required Qualifications
- Graduate of an accredited School of Nursing
- Active RN License/ability to practice in the State of Ohio
- Minimum of 1 year experience as a Registered Nurse
- Minimum of 1 year experience in a home health agency or equivalent setting
- Experience with OASIS and NON-OASIS assessments
- CPR and First Aid Certifications
- Willingness to travel throughout the NW Ohio service area
- Strong documentation and field staff leadership
- 2-5 years of experience preferred
- Nurse supervision/People management desirable
